DDT Care - Supporting Vulnerable Individuals to Achieve Independence

The service aims to provide a safe and homely environment that promotes empowerment, independence, and choice, whilst enhancing our tenants’ daily living skills, enabling individuals to move on to a less supported setting. The age range of people who can access our service is 0-16 years, 16-18 years, and 18-65 years. Our service is available to people from all over the UK

Services
 

Our floating support services (Domiciliary care support) enables us to offer further support to service users moving to their own accommodation, or who have already done so but require support in maintaining their tenancy so they can continue to live independently. This support service is available for as long as the service users requires it and will remain with them even if they move to their home. We therefore offer the following support to service users via the domiciliary care support worker

  • Support ranging from 2 hours – 9 hours per day depending on the service user’s needs.

  • Support where needed with shopping and accessing local amenities.

  • Supervision and monitoring of mental health.

  • Medication administration.

  • Support in setting up and maintaining a home or tenancy.

  • Support in developing domestic skills/life skills and social skills.

  • Support in managing finances and benefits claims.

  • Support with social and leisure activities.

  • Regular risk assessment.

  • Advice/Advocacy and Liaison.

  • Support with gaining access to other allied services.
